The dish is very popular in the Khon Kaen province of Thailand and is known as Koi Pla. It consists of minced raw fish chopped with herbs, spices and lemon juice. It is consumed by millions of Thais, but is particularly popular in one of the country’s poorest provinces, Isan. Now, it’s not the fish itself that causes fatal liver cancer in those who eat it, but rather the parasitic worms that live inside it.

The parasites are native to freshwater fish in the Mekong region and have caused Isaan to have the most high-profile cases of cholangiocarcinoma (bile duct cancer) in the world, because many people there consume the raw fish dish. The disease, nicknamed the “silent killer,” has one of the lowest survival rates of any cancer if not treated with surgery.

A Thai doctor, Narong Khuntikeo, is trying to get fewer people to eat the delicacy after losing both his parents. They died of liver cancer after eating the popular dish. “The health burden here is very high, but no one knows about it because people are dying silently.”

Together with scientists, doctors and anthropologists, Khuntikeo has been examining the parasite in villagers in the Isan region for four years. Using ultrasound machines and urine tests, the doctor discovered that up to 80% of residents in some communities had ingested the deadly parasite. While trying to spread information and warnings about the Koi Pla dish, he has difficulty convincing the older generation who reject his warnings. “Those who oppose the change, despite the health risks, claim that cooking the fish – the best way to kill the parasite – completely ruins the flavor.”
