Officers attempted to break into the building by kicking in doors and windows, but the home was completely barricaded, the Louisville Metro Police Department said.
The unidentified woman managed to break through the window of the second floor room where she was being held.
A neighbor shouted, “I have a ladder!” and footage shows the officer climbing up to the room.
He climbs over broken glass and enters the filthy bedroom before removing the chain from the woman’s neck. He tries to calm her down.
“The woman had a chain around her neck secured with a MasterLock, and that chain was attached to the ground with screws,” police said.
In the video, she is heard crying, apologizing to the police officer and telling him that the man who locked her in kept the key on his keychain.
Two officers used a hatchet they found in the apartment to loosen the chain from the floor.
The woman was in shock and continued to shake and cry as firefighters used bolt cutters to remove the locked chain from her neck.
She was helped outside and treated by paramedics.

May was later arrested after the woman was rescued. He is said to have held her captive in the house after an argument turned physical.
According to an arrest report by WAVE 3, he held her in the bathroom and cut off most of her hair with a machete.
She left that same night and came back the next day to collect her things, after which he locked her in the room.
“You’ll get it tonight.” “I told you that the next time you leave and don’t come home, I’ll kill you,” May reportedly told her.
He is said to have forced her to undress and slapped her in the face. He is said to have stormed out of the house after chaining her to the floor and taking away her cell phone so she couldn’t call for help.
