Not only does he have to share half of his fortune with his exwife, but he also has to deal with an investigation into a patient's death that could lead to his registration being revoked. In 1993, Augusto suffered this punishment due to the death of a child.

Infertility was added into the story to modernize the plot. In the original version, José Augusto got involved with Buba (Gabriela Medeiros) after the death of Venâncio (Rodrigo Simas) and the “blame” for not giving José Inocêncio grandchildren fell on the character, who was intersex in 1993. In the current soap opera, the impossibility will also be that of the doctor.
